What is Disc Injury? What is it and What Causes it?

The Intervertebral Disc

Disc is a sophisticated, cushion-like gel structure located between two vertebrae (spine bones). It functions as the spine’s primary shock absorber, allowing for the complex movements we perform every day.


Structurally, the disc is composed primarily of water and collagen fibres (collagen fibre 1 and 2), organised into two distinct layers:


1.        Annulus Fibrosus (The Outer Protective Ring)

Consists of 12-15 layers of tough collagen fibres arranged in a specialised crisscross pattern. This design provides immense structural strength, acting like a radial tire to contain the inner core and mange spinal pressure

 

2.        Nucleus Pulposus (The Inner Core)

This is the “gel-like” centre of the disc. It is highly hydrated and resilient, and its main function is to distribute weight evenly across the vertebral segment and absorb the impact of gravity and movement.

Why Do Disc Injuries Occur?


The disc is a robust structure but has specific mechanical vulnerability. Biomechanically, the disc is most vulnerable during combined flexion (bending forward) and rotation (twisting). That’s why many people experience sharp sudden pain in the lower back while lifting or bending over to put the shoelace on.


Common causes of injury include:

  • Acute Trauma: Sudden force such as motor vehicle accident, a fall, or heavy lifting with poor technique


  • Repetitive micro-trauma: A small tear or injury in the outer layers (Annulus fibrosus). While small, these can be highly painful due to the rich nerve innervation in the outer ring


  • Disc Protrusion (Bulging Disc): When the inner gel pushes against the weakened outer ring, causing it to “bulge” into spinal canal.


  • Degenerative Disc Disease (DDD): A gradual process where the disc loses its water content and height, reducing its ability to absorb shock and often leading to stiffness and pain.
